I just looked at the Old Testament[1] again. 
The hello world program is on the second page of the first 
chapter. Sometimes the book does something, then explains 
the theory later. Sometimes theory comes first. In any case, 
they are never far apart. 

As mentioned by Steve Roggenkamp, K&P's "The UNIX Programming
Environment" is another excellent book. 

Did Kernighan, Ritchie, or Pike write any bad books? 

For knock-down drag-out hard core reference, 
W. Richard Stevens was excellent. Read APUE. 

[1] "The C Programming Language" has two editions. 
    The first edition is also called the "Old Testament", 
    and the second edition is also called the "New Testament". 

[2] Advance Programming in the UNIX Environment
    (there is a second edition updated by another author, 
    Rago, who continued the precedent of fine writing set by 
    Stevens.)
