As to submission, and then SMTP of pieces in and out, sendmail 
runs a TLS layer with minimal effort (check the headers of 
this piece)

As to retrieval, both dovecot and the cyrus suite, also do 
TLS

We set up turnkey-appliances for medical office clients which 
need to attend to HIPAA issues, and solved this long ago

'horde' is out there but is rather a moving target of large 
size, so packaging rots, and well enough packaged elsewhere

which I have maintained for a long, long time to satisfy GPL
