Support 360 Special Initiative on Emergency Transition by the Department of
Health and Human Services. The goal of the project is to develop an
implementation plan for a one-stop center on emergency transition for
individuals with developmental disabilities and their families. Emergency
transition can be defined as any instance when a person with a developmental
disability is dislocated from their place of residence due to manmade or
natural disaster, or loss of a primary caregiver. The implementation plan will
be based on best practice research, input from individuals with developmental
disabilities and their family members, research on information technology
methods and systems, and barriers to successful transition. The project will
eventually provide resources to individuals with developmental disabilities and
family members so they can prepare for emergency transitions, remain intact as
a family and avoid unnecessary institutionalizations. <o:p></o:p></span></font></p>
