Support 360 Special Initiative on Emergency Transition by the Department of
Health and Human Services. The goal of the project is to develop an
implementation plan for a one-stop center in Franklin County, Ohio, on
emergency transition for individuals with developmental disabilities and other
functional needs and their family members. Emergency transition can be defined
as any instance when a person is dislocated from their place of residence due
to manmade or natural disaster, or loss of a primary caregiver. The
implementation plan will be based on best practice research, input from
individuals with developmental disabilities and their family members, research
on single-point of entry technology and systems, and identified barriers to
successful transition. The project will eventually provide resources to individuals
with developmental disabilities and family members so they can prepare for
emergencies and remain intact as a family and avoid unnecessary
institutionalizations.<o:p></o:p></span></font></p>

independent environment as possible. The vision of the project is all
individuals with functional needs in <st1:place w:st="on"><st1:State w:st="on">Ohio</st1:State></st1:place>
will have emergencies transition plans in place that will make for a smooth
transition during a personal or community emergency. <o:p></o:p></span></font></p>

who serve individuals with functional needs, and emergency management personnel
