Family Support 360 Initiative: Emergency Transition

Planning and Advisory Committee Request for Proposals

 

Proposal for Information Technology Research

 

Background

Ohio was recently awarded a Family Support 360 Special Initiative on
Emergency Transition by the Department of Health and Human Services. The
goal of the project is to develop an implementation plan for a one-stop
center in Franklin County, Ohio, on emergency transition for individuals
with developmental disabilities and other functional needs and their
family members. Emergency transition can be defined as any instance when
a person is dislocated from their place of residence due to manmade or
natural disaster, or loss of a primary caregiver. The implementation
plan will be based on best practice research, input from individuals
with developmental disabilities and their family members, research on
single-point of entry technology and systems, and identified barriers to
successful transition. The project will eventually provide resources to
individuals with developmental disabilities and family members so they
can prepare for emergencies and remain intact as a family and avoid
unnecessary institutionalizations.

 

The mission of the single-point of entry (SPE) is to provide people who
have developmental disabilities and other functional needs, as well as
family members and other caregivers, with the supports they need during
a personal or community emergency so they remain in the least
restrictive and independent environment as possible. The vision of the
project is all individuals with functional needs in Ohio will have
emergencies transition plans in place that will make for a smooth
transition during a personal or community emergency. 

 

This RFP seeks a consultant to assist the Planning and Advisory
Committee of the project to make an informed decision about the
technology and design needs of the SPE. The consultant will provide
research, information and recommendations to the committee on building a
system that meets the needs of the target audiences - individuals with
developmental disabilities and other functional needs and family
members, service providers who serve individuals with functional needs,
and emergency management personnel and first responders.

 

The maximum amount for this contract is $14,500. The completion date for
this contract is June 15, 2010.

 

Scope of Work

The contractor will provide information and recommendations on a SPE
system design so the Planning and Advisory Committee can develop a
detailed implementation plan for the SPE. The intent is to have a
multi-layered access SPE that includes a web-based interactive system
that will allow users to access information, tools and training without
having to physically visit the center. The website would have links to
information and referral systems, service providers, and the larger
emergency management community.

 

The contractor will:

*    Research current SPEs in Ohio and in other states where similar
programs are using SPE technology

*    Provide information on technology approaches that would meet the
functional needs of the web-based system including-

-          The ability of target audiences to access information, tools
and other resources to increase their ability to plan for emergency
transitions. Content would be provided by project staff.

-          Allow for interactive discussion within the disability
community through social media

-          Link users with the websites of service providers who can
assist in an emergency

-          Allow users to sign up for a reminder email

-          Have contact information for the project staff

*    Investigate way to connect the SPE system with existing I&R systems
and other resources related to emergency management in Franklin County.
Provide sample language for service level agreements.

*    Make recommendations to the committee on which method would work
best for the SPE and provide cost information on purchase or
development, set-up and maintenance requirements

*    Provide recommendations on data collection methods for the SPE
website so the project can identify usage and satisfaction with the
information and services provided by the project.

Deliverables

This contractor will provide the Planning and Advisory Committee with
the following deliverables by the end of the contract period, with the
exception of the meeting described below.

 

1.      Written report on the results of the research outlined under the
Scope of Work and recommendations of the contractor.

2.      A list of information and referral systems and emergency
management systems and recommendations on how to integrate the SPE with
those systems.

3.      In person consultation with the Planning and Advisory Committee
at the June 17, 2010 meeting to answer questions and review
recommendations.

 

Proposal Content Requirements

The proposal should include the following, be detailed and succinct and
have a maximum length of 3 pages.

A.	Contact person for the proposal and contact information.
B.	Background and specific experience in ways the contractor has
performed similar functions in the past. Provide information on
information technology skills and experience in providing consultation
and recommendations to clientele.
C.	A clear explanation of how the contractor will go about the
Scope of Work detailed above.
D.	References and contact information on similar projects completed
by the contractor.
E.	Describe the contractor's capacity to carry out the Scope of
Work in the short period of time available.
F.	Project cost.

Decision-Making Process

The Planning and Advisory Committee will be making a decision and
awarding a contract by April 5, 2010. The successful candidate must be
able to quickly get the research project started and completed by the
target date of June 15, 2010.

 

Proposals will be evaluated on the contractor's expertise and experience
in similar projects, the ability to work with the planning committee,
and the detailed project plan submitted in the proposal. Any proposal
that exceeds the contract amount will not be considered.

 

The Family Support 360 Special Initiative on Emergency Transition
Planning and Advisory Committee reserves the right to reject any and all
